To schedule surgery for your pet, please fill out the form.  We will respond within a few days with additional information.

24 hours prior to your pet's surgery, please print your Presurgical Instructions and Surgery Release forms.  Please bring the Surgery Release form with you.  You may download the form by clicking the PDF link below:

WE WILL BE RESUMING SURGICAL SERVICES STARTING MAY 27TH
We will have 14 surgical days available.  All pets over 6 years of age need pre-surgical labwork no longer than 6 weeks prior to surgery.

Please Note:
WE MUST SEE YOUR PET BEFORE WE ARE ABLE TO SCHEDULE THEM FOR SURGERY.

PLEASE NOTE
FOR MOST ALL SURGERIES, WE ARE BOOKING 3-4 WEEKS IN ADVANCE
All pets must have pre-surgical lab work done prior to being put onto our dental schedule.  We required a comprehensive chemistry panel and a CBC.  This lab work may be done during a standard appointment and must be no longer than 6 weeks prior to the surgery date.
