Online Prescription Order Form
​All patient prescription requests must have been seen by the doctor within the year, or sooner if doctor requests. There is a $8 shipping fee for all online orders. Payment must be received prior to shipment. Thank you for your understanding. Please contact info@affordablepetvaccines.com for further questions. 

Requests will be received during Open business hours. We will then send you a PayPal Invoice for your amount due. Once paid in full, your medications will be mailed out within 24 hours. Please note there is a $8.00 shipping fee for all medications. Thank you for your understanding.
